The Health Center is open 24 hours per day during the school week. We provide nursing services to students with minor illness/injury ONLY. Any student who is seen and cannot be kept at school safely will be sent home. If your child has any of the following, he/she cannot stay at school or if your child has these symptoms at home, please do not send them to school:
- Fever of 100.0 or more
- Vomiting with or without fever
- Diarrhea with or without fever
- Head Lice or Nits
- Suspected Pink-Eye (Conjunctivitis)
- Open sores that cannot be covered by a Band-Aid
- An undiagnosed/untreated rash
This helps us greatly reduce the spread of infectious illnesses to our students and staff. It is the parent’s responsibility to provide transportation to pick up their sick child. KSD is not responsible for transportation or making arrangements with your child’s school district when they are sick. Parents will be called and given a timeline for pick up. If a child is not picked up by the parent/guardian or if transportation for that child is not arranged, social services will be notified.
A Medical Release form will be provided to you when your child is picked up, with specific information regarding the cause of the release, and how many days the student’s absence will be excused without a Doctor’s note. Any absence beyond the date given on the form will be unexcused until such time as we receive a Doctor’s note.
He/she may not return to school until they are well (no fever, no vomiting, no diarrhea), without medication for a full day (24 hours unless otherwise noted not to be contagious by a physician, such as with an ear infection).
Doctor's excuses must include the date seen and the date to return to school. They should also include any restrictions or instructions related to school.
All suspected cases of conjunctivitis (pink-eye) must be referred to a healthcare provider.
All bacterial illnesses determined to be contagious must be treated with antibiotics for a minimum of 24 hours before student may return to school.
SEE CHART BELOW WITH OTHER COMMUNICABLE DISEASE AND EXCLUSION PERIOD
If your child requires medication on a daily basis the following will apply:
- Signed consent from the parent/legal guardian to administer prescription medication.
- All medication must be in its original container and dated within the last 90 days.
- Prescription meds will be given as ordered by the physician on the container. Changes to prescriptions by the physician must be faxed to 859-936-6656.
- Medication will no longer be transported home every weekend and will be stored in locked containers in the health center. Medication may be sent to school in one of two ways: (A) Send a 5 day supply and the empty bottle will be returned on the weekend; or (B) Send a month’s supply and keep what you need for the weekends/holidays at home.
- Medication refills are the responsibility of the parent. We will send the bottles home for refills when we have a week supply left.
- New prescription medications will require a note explaining the need for the medication and permission to administer the medication.
- If your child is sent to school without their prescribed medications and this causes safety issues, you will be asked to pick up your child.
If your child requires any OTC (over the counter) medication, the following will apply:
- ALL OTC (over the counter) medication MUST be provided by the parent/legal guardian. This includes substances such as Neosporin, eye drops, cough drops, hydrogen peroxide, etc. KSD cannot administer ANY over the counter medicinal substances to students.
- Signed consent from parent/legal guardian to administer OTC medication, and a note explaining why your child needs the medication. (If you already provided signed consent and this information on the registration form, you do not need to fulfill this step again)
- Medication MUST be appropriate for your child’s age / weight
- Write your child’s name on the box or container
- OTC medication cannot be given for more than 5 consecutive days without a physician’s order
Parents are responsible for all medical, dental and vision appointments. Please take time this summer to get all required exams completed before registration day.
*Reminder of requirements*
- Immunizations: Every KSD student must have a current Kentucky State Immunization Certificate on file at school. Medical and religious exemption certificates are available.
- Physical Exams: Students need proof of a physical exam completed on a Kentucky Preventative Exam Form prior to their initial (first) entry into school and for sixth grade. This exam is not the same as a sports physical. If your child participates in any athletic program at KSD, a sports physical is also required, and must be completed on the KHSAA forms in KSD registration book.
- Vision Exams: Students ages 3, 4, 5, or 6 years old entering KSD for the first time must have an eye exam completed by January 1 of the year the student is enrolled. This is a one-time-only requirement. This exam must be completed by an ophthalmologist or optometrist and is not part of the physical exam. Proof of the exam must be submitted on the Kentucky Vision Examination Form.
- Dental Exams: Students ages 5 or 6 years old entering KSD for the first time must have a dental exam completed by January 1 of the year the student is enrolled. This is a one-time-only requirement. Proof of the exam must be on the Kentucky Dental Screening/Examination Form.
*ALL new students (initial school enrollment and students transferring from a different district / school) MUST provide proof of a current Kentucky state immunization certificate, and physical, vision, and dental exams.
